Coding error behind missing child protection reports in Queensland

[1] According to the findings of a Deloitte investigation, a simple but undetected coding error is to blame for the failure of Queensland’s OneSchool system to successfully transmit hundreds of abuse reports.

[2] The report was publicly released today by Education Minister Kate Jones, which was triggered by a software upgrade to the mandatory child protection reporting module of OneSchool early this year.

[3] The software failure resulted in the disappearance of up to 1000 serious messages from teachers and principals.
